LEAVE number 1 blank on the left side if your Command does not have a TC 0-6 

Nominator block must be filled out

Planning is a must ..leave the appropriate mailing time to get to the TCRA, processing time or returning mailing time, so it can be returned for presentation date

Presentation date is the date you want on the certificate
